When writing out an address that includes a street number, the words “avenue,” “boulevard” and “street” are always abbreviated: 

1600 Pennsylvania Ave., 2020 Glendale Blvd., 526 Wall St.

When referring to avenues, boulevards and streets without using a street number, the words are spelled out:

Pennsylvania Avenue, Glendale Boulevard, Wall Street

All other words that denote roadways (drive, place, court, road, etc.) are always spelled out in addresses:

1313 Rodeo Drive, 4250 Peachtree Road, 405 Peyton Place
